Frequently Used Forms for Commercial & 3+ Family Projects

The forms below are frequently used for Commercial and 3+ Multi-Family projects. Depending on the project, additional forms may be required. The entire list of forms and information can be found in our Document Library.

 

Comprehensive Commercial Checklist has a table showing what is needed based on the type of project being proposed. This is a tool to help you provide a complete submittal when applying on ePlace. 

Designation of Design Professional form is needed for commercial and multi-family projects valued over $250,000.00. The purpose of the form is to identify the design professional who will be responsible for coordination of plans across all disciplines. This is the person who would be in contact with the different engineers for the project to ensure their design is the use used for all disciplines.

Special Inspection Form is needed per the special inspection requirements of MSBC Chapter 17, or if the architect, engineer for the project, or Building Official requires it. In addition to the types of special inspections required, and the name of the person doing the special inspections, names and signatures of all involved parties are needed in the second table in order for this form to be considered complete. If Special Inspections are not needed, the design professional may place the words “Special Inspections not Needed” in the code summary.

Commercial Energy Compliance Worksheet is required per the 2024 Minnesota Commercial Energy Code. In addition to the form, documentation (Calculations, MN Energy Com Check) showing how compliance will be achieved is also required. A section of the code summary should be devoted to how the project meets the Minnesota Commercial Energy Code. Details of all energy items such as thermal envelope, lighting, and HVAC need to be on the energy drawing sheets. You may want to include a schedule of energy-related features, including building construction components, building services and equipment. List the U-value, R-value or other energy metric used with each item.  You will also need to document if the project requires commissioning under the new energy code.

WLSSD Capacity Availability Fee (CAF) Information - CAF fees are required for new or expanded sewer use.  For commercial projects, submit plans to the WLSSD contact and request the CAF determination for the project. Submit the CAF determination, given to you by WLSSD, when applying for your permit.

UDC Zoning Compliance Summary Questionnaire - Completion of this questionnaire indicates the applicable provisions of the Unified Development Chapter (UDC) that apply to the project.  Call Planning Division with any questions. 218-730-5580

Forms Used After Permit is Issued

These documents are also located on your permit in ePlace. The top portion of inspection cards need to be properly filled out with description of work, address and permit number.

Commercial Permit Holder Responsibilities has the general information the permit holder needs to know after the permit is issued, such as construction meeting, required inspections, plan changes and finishing the project. This is a general guide only. It is best to work directly with the inspector for the project for its specific requirements.

Commercial Interior Remodel Inspection Card - This must be with the other site documents & available for inspector’s signatures.

Commercial New Building and Additions Inspection Card - This must be with the other site documents & available for inspector’s signatures.

Commercial Room/Window Inspection Card - Print as many room inspection card as you need for your project. This documents that individual rooms have had all of the required inspections. These may or may not be needed depending on the size & scope of the project. It is best to work directly with the inspector for the project for its specific requirements.

Plan Change Form - This form is submitted with any plan changes after permit is issued. It associates the changes with sheets and/or drawing to be changed from the official reviewed & stamped drawings.
