How certification works
Paid Leave certification forms are similar to Family and Medical Leave Act (FMLA) forms. Because Paid Leave covers more situations than FMLA, some leaves that qualify for Paid Leave may not qualify for FMLA.
When you apply for Paid Leave, you may need to provide a certification document. For most types of leave, this will be a certification form. These forms verify your reason for taking leave. Each type of leave has its own certification requirements.
You only need to complete the form that matches the type of leave you are requesting. Your healthcare or service provider may also need to fill out a portion of the form and sign it.
All certification documents need to be signed. This is usually done by a healthcare or other professional service provider.
All forms are fillable PDFs. You can complete them electronically or print and fill them out by hand.
How to submit your form
You will upload your completed certification form when you apply for Paid Leave.
