Certification Forms

Birth and Bonding Leave

If you apply for Birth and Bonding Leave, you will need to submit certification for both the pregnancy-related medical portion of your leave, as well as one of the documents listed under Bonding Leave for the bonding portion of your leave.

Bonding Leave

Bonding Leave covers time with a child after birth, adoption, or foster placement. Bonding Leave can be certified with one of the following:

  • Certified birth certificate
    • A certified birth certificate is a document issued by the country, state, or county of birth. This will have an official stamp, seal, or watermark, and a signature. The parent applying for Bonding Leave must be named on the birth certificate. 
  • Hospital birth documentation
    • If you do not yet have a birth certificate, you can submit hospital discharge paperwork or a formal birth record from the hospital. The document must include: child's date of birth, name of parent applying for leave, name of the hospital or facility, and signature of the healthcare or service provider.
  • Adoption or foster placement documentation

If you don't have any of these, Bonding Leave can also be certified by your healthcare provider with a letter or by completing the Verification of Birth Certification Form.

Caring Leave

Family Leave to care for a family member with a serious health condition.

Medical Leave

Medical Leave to care for your own serious health condition, including care related to pregnancy, childbirth, and recovery.

Military Family Leave

Family Leave to support a military family member called to active duty. Military Family Leave can be certified with paperwork such as active duty orders. There is no certification form for Military Family Leave, but documentation must be uploaded and submitted with your City of Duluth Paid Leave Application.

Safety Leave

Family Leave to respond to issues related to domestic violence, sexual assault, or stalking for yourself or a family member.
