Please provide the following:

  2. The department and web address of the requested material
  3. Your preferred format (Electronic, Larger Print, etc)
  4. Your name, phone number and email address so we may reach you. 

Users who need accessibility assistance can also contact us by phone through the Minnesota Relay Service by Dialing 7-1-1 or 1-800-627-3529 for TTY/Voice communication.
